Terms and Policies

NY Telehealth Acknowledgment:

I acknowledge that psychotherapy services with Dr. Joyce Yung, Ph.D. are provided via telehealth and are available only to individuals who are physically located in New York State at the time of each session. I understand that I am responsible for confirming my location and that services may be paused or discontinued if I am not located in New York State, in accordance with licensing regulations. I understand that there are risks and consequences from telemedicine, including, but not limited to, the possibility, despite reasonable efforts on the part of my psychotherapist, that the transmission of my medical information could be interrupted or distorted by technical failures or unauthorized persons, and that the electronic communication of my medical information could be accessed by unauthorized persons. I understand that telemedicine based services and care may not be as complete or effective as face-to-face services. I also understand that I am responsible for ensuring that I am in a private and secure environment during sessions to protect my own confidentiality. I acknowledge that while reasonable safeguards are used, no digital or internet-based communication can be guaranteed to be completely secure. I understand that I should take appropriate precautions on my end, including using secure internet connections and a private space whenever possible.

I also understand that if my psychotherapist believes I would be better served by in-person psychotherapeutic services, I will be referred to a psychotherapist who can provide such services in my area. I understand that there are potential risks and benefits associated with any form of psychotherapy, and that despite my efforts and the efforts of my psychotherapist, my condition may not improve, and in some cases may even get worse. I understand that I may benefit from telemedicine, but that results cannot be guaranteed or assured. As with all medical records, I understand that I have a right to access my medical information and copies of medical records of telemedicine treatment in accordance with New York State law. I further understand that telehealth services are not intended for emergency or crisis situations, and I agree that I will seek immediate support through appropriate emergency services if I am in crisis.

Consultation Disclosure:

I understand that this complimentary 20-minute consultation is an introductory conversation to explore my goals, answer general questions, and assess fit. It is not psychotherapy, diagnosis, or treatment, and does not establish a therapeutic relationship. Any insights or suggestions shared are preliminary, based on limited information, and may change if therapy begins. This consultation does not include emergency or crisis support. I understand that participation is voluntary and that I may choose not to continue after the consultation without any obligation. I also understand that no specific outcomes or results are guaranteed from this consultation.

General Consent/Privacy Policy:

I understand that psychotherapy services with Dr. Joyce Yung are available only to individuals physically located in New York State at the time of services. I understand that my personal information is handled with strict confidentiality in accordance with applicable laws and professional ethical standards, and that information may only be shared with my explicit consent or as required by law. I acknowledge that privacy protections apply to both in-person and telehealth services, but that digital communication carries inherent limitations as previously described.

By scheduling and participating in the complimentary 20-minute consultation, I acknowledge that I have read, understood, and agree to the terms outlined above.
